Facial Palsy, Hypoacusis and Vertigo as Inaugural Symptoms of Neuroborreliosis - Case Report

Introduction: Facial Palsy (FP) is the most common acute mononeuropathy and consists of a decrease in facial muscle strength due to nerve damage. Peripheral FP (PFP) can result from wide variety disorders aetiologies, with Lyme Disease (LD) being considered one causes FP. LD an infectious disease that affects central nervous system, causing Neuroborreliosis (NB) 15% cases. Cranial neuropathy form presentation NB, as uni or bilateral PFP. We describe case PFP hypoacusis vertigo inaugural neuroborreliosis. Case Report: present report 75-year-old female patient, referred ENT consultation 3-day course grade 4 right PFP, moderate sensorineural hearing loss vestibular hypofunction. The patient underwent cranioencephalic Computed Tomography (CT-CE) which excluded event was treated 2 cycles oral corticosteroids, without any clinical improvement. A Magnetic Resonance Imaging (MRI) performed, showed abnormal inflammatory uptake nerve. An analytical positivity for Borrelia IgM found diagnosis polyneuropathic NB involvement VII VIII cranial pairs assumed. completed 28 days doxycycline, improvement normalization acuity. Discussion/Conclusion: In presented case, absence classical migratory erythema painful meningopolyneuritis didn’t exclude NB. associated signs other nerves raised hypothesis systemic disease, motivated etiological investigation carried out.
